Making Every Contact Count in Health and Care

A new service is being designed to allow health and care staff in City & Hackney to:

• have conversations about non-medical issues affecting people’s health,
• give brief advice and signpost people to other sources of information/advice and support they may need.

This is a great opportunity for residents to have a real say in the design of this service with the commissioners designing the service.
